REGISTRATION/WAIVER FORMS FOR OUR FALL SEMESTER AND SHOW!

You must complete both forms to register your child into our musical theater training program.  

Please make sure you include your child's emergency contact info, and contact pickup person if applicable.

Completed forms and payment will confirm admittance into the musical theater program.  You will receive an email confirmation within one day of registration.  

POLICIES AND WAIVERS

PAYMENT FOR TRAINING/PRODUCTION


Student must fill out a completed and signed registration form with full payment at the time of registration and before classes begin.

Break A leg Productions, LLC accepts cash payments, checks, Venmo, Paypal (with payment plan), Visa and Mastercard, and American Express. A $20.00 fee will be assessed on all returned checks.


POLICIES

  • Student alcohol and drug use is strictly prohibited.
  • Student smoking is strictly prohibited.
  • BAL’s receipt of completed application and payment guarantees student acceptance into programs.
  • BAL is not responsible for students who do not attend classes or activities as scheduled. If a student leaves BAL’s premises or supervised activities, BAL is no longer responsible for his or her whereabouts, actions or welfare. BAL is not responsible for students during their free time outside our program hours.
  • Students will have very limited free time during programs and are expected to prepare for classes during off time. 
  • Parents or guardians must inform BAL of any pre-existing medical conditions of a student. This must be done in writing via a series of online forms. No students will be permitted to begin the programs without medical and insurance information and a parent signature. 
  • Violation of any BAL Standards and Regulations may result in exclusion from the programs without refund of tuition.
  • Student and/or parent/guardian shall hold BAL and its staff harmless for any injury that may occur.
  • All photographs, video and/or audio recordings of the students by BAL may be used in connection with the promotion, publicity, and marketing of the Break A Leg Production's Musical Theatre website and Intensive programs without permission or compensation to the students.
  • Break A Leg Productions does not discriminate based upon gender, race, ethnicity, or other physical characteristics.
  • If a student misses any portion of a Workshop or Intensive for any reason (vacation, industry business, medical emergency, etc.) BAL will do our best to accommodate them so that they can be in the   final showcase participation, but there may be exceptions.
    • No program will be pro-rated if a student is unable to attend any portion of the program.

REFUNDS


This training /production program is a two-week full day (10-3pm program. If a student withdraws from a class two weeks prior to the start of the class, there will be a full refund minus a $35.00 cancellation fee. Thereafter, there are no refunds for withdrawals from the class/show.


Break a Leg Productions, LLC reserves the right to cancel a class or show if insufficient enrollment. Parents will be immediately notified if class is cancelled and will get a full refund.


MAKEUP LESSONS


There are no makeup classes for this training production.  We encourage our students to attend every class to ensure that they feel prepared and gain the full benefit of the musical theater training program. Additional classes/rehearsals may occur close to production time, and is included in the tuition and full payment of the program.

CANCELLATION


Break A Leg Productions LLC reserves the right to cancel any class due to insufficient enrollment and to close a class when it reaches capacity of 40 students.


In the event of inclement weather, or in conjunction with the closing of WILTON public schools and early dismissal due to inclement weather, we may cancel a class if the teacher is unavailable, or the roads are dangerous to travel.  You can call 203-216-0995 or check our website for cancellations. In that event, we may arrange a makeup lesson for the program contingent on inclement weather.


WAIVER/PHOTO RELEASE


Break a Leg Productions, LLC seeks permission to take photographs, videos, films, recordings, electronic images, tapes of  my son and/or daughter while participating in and/or attending programs, classes, rehearsals, performances.
